Why Was My HSA Card Declined? Eligibility, Balance, and Card Issues

An HSA card gets declined most often for one of five reasons: the item isn’t a qualified medical expense, your available balance is too low, the card itself is expired or on hold, the store’s payment system can’t confirm the purchase is eligible, or your administrator is waiting on a receipt from an earlier transaction. A decline at the register rarely means your money is gone. If you’re wondering why your HSA card was declined, the fix is usually quick, and in the meantime you can pay another way and reimburse yourself from the account later.

The Item Isn’t HSA-Eligible

The IRS limits HSA spending to medical care: costs for diagnosing, treating, or preventing illness, or affecting a structure or function of the body. Since the CARES Act took effect in 2020, over-the-counter medications and menstrual care products qualify without a prescription.2Internal Revenue Service. Insurance Premiums

Most health insurance premiums cannot be paid with HSA funds, which surprises many cardholders. The IRS allows only a few narrow exceptions:4Internal Revenue Service. Publication 969 (2025), Health Savings Accounts and Other Tax-Favored Health Plans

  • COBRA continuation coverage
  • Health coverage while you receive federal or state unemployment benefits
  • Medicare Parts A, B, C, and D premiums if you are 65 or older (but not Medigap)
  • Long-term care insurance premiums, up to an age-based annual limit

Any other premium, including your regular monthly health plan premium, will cause a decline.

Dual-Use Items Without a Letter of Medical Necessity

Some products straddle the line between medical and personal use, such as a mattress for chronic back pain or an air purifier for severe allergies. Your HSA administrator may approve these purchases if you submit a letter of medical necessity from your doctor. Those letters are typically valid for up to 12 months, after which you need a new one. Without a current letter on file, the card will be declined for these items.

Your Available Balance Is Too Low

Your HSA provider distinguishes between total balance and available balance. Payroll deductions and personal check deposits often take one to three business days to clear, and the card won’t work against funds still in that clearing window even if the total balance shows them.

HSA providers don’t offer overdraft protection or credit lines. If the bill is $150.00 and your available balance is $149.95, the system rejects the whole transaction rather than processing a partial payment. Check the “available for withdrawal” figure in your provider’s mobile app before you present the card.

Something’s Wrong With the Card or the Account

Even with enough money and an eligible purchase, administrative issues with the card itself can block a transaction.

  • A new card that hasn’t been activated by phone, website, or app
  • An expired card whose printed date has passed
  • A wrong PIN, or no PIN set up at a terminal that requires one
  • A fraud freeze triggered by unusual spending patterns; call the number on the back of the card to verify recent activity and lift the hold
  • An identity verification hold when your provider has asked for updated personal information under federal anti-money-laundering rules and hasn’t heard back5Financial Crimes Enforcement Network. USA PATRIOT Act
  • A damaged magnetic stripe or cracked chip the terminal can’t read

You Have an Outstanding Substantiation Request

HSA administrators are required to verify that distributions were used for legitimate medical purposes. Many transactions clear automatically through the store’s approval system, but others require you to submit an itemized receipt. When the administrator can’t verify a past purchase, they send a documentation request, and if you don’t respond within the required window (often 30 to 40 days) the card can be deactivated temporarily.

The hold stays in place until you upload the documents or return the unverified amount to the account. A decline today can trace back to a missing receipt from weeks ago, so check your provider’s portal for pending requests if the card is unexpectedly blocked.

Employment or Coverage Changes

If you leave your job, switch to a non-qualifying health plan, or enroll in Medicare, you can no longer make new contributions. The money already in the account is still yours to spend on qualified medical expenses, with no deadline to use it.6Healthcare.gov. Contact the provider to confirm whether the card can still be used for distributions, or withdraw funds manually to pay yourself back for out-of-pocket expenses.

The Store’s System Can’t Confirm the Item Is Eligible

Even a legitimate medical product can be blocked if the store’s technology doesn’t recognize it. Two systems screen HSA transactions at checkout, and a gap in either one causes a decline.

Inventory Information Approval System

Most large retailers and pharmacies use an Inventory Information Approval System (IIAS) that checks each item’s barcode against an approved medical product list maintained by the Special Interest Group for IIAS Standards.7Special Interest Group for IIAS Standards. Eligible Product List Overview If a barcode isn’t on the list, or the store hasn’t implemented IIAS at all, the terminal can’t confirm the item qualifies and blocks the card. This happens often at smaller retailers or stores that recently added health products.

Merchant Category Codes

Every business is assigned a four-digit merchant category code identifying what it sells. Your HSA card is programmed to work only at merchants with healthcare-related codes. Swipe it at a gas station, grocery store, or clothing retailer and the transaction is blocked regardless of what’s in your basket. A gas station that happens to sell bandages still reads as a fuel provider to the payment network.

What to Do When the Card Is Declined at Checkout

You don’t have to walk away from the purchase. Pay with a personal credit card, debit card, or cash, then reimburse yourself from the HSA afterward. There’s no federal deadline to request reimbursement. You can pay yourself back days, months, or years later, as long as the expense was incurred after you opened the HSA and qualifies as a medical expense.

To process the reimbursement, log into your HSA provider’s portal or app, submit a request, and upload supporting documentation. Providers typically want:

  • An itemized receipt showing the provider or store name, date, and description of the service or product
  • An Explanation of Benefits from your health plan, if applicable
  • For prescriptions, a pharmacy receipt showing the drug name and purchase details

Credit card statements and canceled checks generally aren’t accepted because they don’t describe the service. Keep the original receipts.

If the Purchase Really Wasn’t Eligible

If HSA funds went toward something that doesn’t qualify, the distribution is added to your taxable income for the year and hit with an additional 20% tax, reported on Form 8889.4Internal Revenue Service. You may be able to avoid the penalty by returning the money as a “mistaken distribution.” The IRS allows this when the distribution happened because of a reasonable error, such as not realizing a product was ineligible. The repayment must be made no later than the tax-filing deadline (not including extensions) for the year after you first knew or should have known about the mistake.9Internal Revenue Service.
