How to Request Your Report Online
Go to the ChexSystems Consumer Disclosure page and choose the option to log into the Consumer Portal or create an account.2ChexSystems. Consumer Disclosure After you enter your identifying information, the system runs a real-time search of its database and then asks a short series of multiple-choice identity verification questions drawn from your financial history. Expect questions about past account relationships or previous addresses.
Answer the questions correctly within the time the portal gives you. If verification succeeds, your report generates immediately as a downloadable PDF you can save or print. The consumer score report is a separate product — a numerical risk score, not the disclosure itself — and you can request it through the same portal.
What You Need Before You Start

The name and address you enter should match what your bank has on file. Even minor mismatches can keep the system from locating your record.
If Online Verification Fails
The portal does not simply reject you when the questions cannot confirm your identity. Your request shifts to a manual review, and ChexSystems mails the report instead. When You Can Get Extra Free Copies
The annual copy is not your only free look. What the Report Shows
A ChexSystems disclosure is about deposit accounts, not credit cards or loans. It focuses on checking and savings history and is organized into several sections:6ChexSystems. Sample Consumer Disclosure Report
- Reported information: accounts closed with a negative balance, including the reporting bank, the reason for closure (such as unpaid overdrafts or suspected fraud), the closure date, the original amount owed, and whether the balance has been paid.
- Inquiries viewed by others: banks or credit unions that pulled your file, typically when you applied for a new account.
- Retail information: returned checks reported by merchants or other businesses.
- Check ordering history: a log of when and where checks were ordered under your name.
- Security freeze and fraud alert status: whether any freeze or alert is currently active on your file.

Disputing Errors After You Read It
Once you have the report, read it carefully. If you spot something wrong — an account closure you do not recognize, or a debt listed as unpaid that you already settled — you have the right to dispute it. ChexSystems accepts disputes online through the Consumer Portal, by phone at 800-428-9623, or by mail.4ChexSystems. ChexSystems must complete a reinvestigation within thirty days of receiving your dispute.
